  • This form will take 20 minutes to complete. Before commencing, please note the student’s MGGS reference number and ensure that you have scanned copies of relevant documentation such as the student’s Immunisation History Statement, Health Action plans, Visa and Passport.  If you close the form, your device will save your progress and you will be able to complete it at a later date.

  • Melbourne Girls Grammar (“MGGS”, “the School”) requires confirmation of student details as an essential part of the enrolment process at MGGS.

    Students are not permitted to commence at MGGS without a completed Confirmation of Student Details form being provided to MGGS.

  • The Year Level Parent Directory exists to help parents feel a sense of belonging to the MGGS community. Sharing details assists our Parents Association welcome new members of the community, and to invite you to social events for your daughter's year level/s or broader MGGS activities. It also helps you to liaise with other parents, organise catch ups for your daughter, or even carpool to or from School. Your email address is shared with your daughter’s year level only.

  • EMERGENCY CONTACTS

  • Please list two emergency contacts for use if the Parents/Guardians are not able to be contacted.

    Emergency Contacts must live in Melbourne and surrounding areas.

    Emergency contacts are authorised to:

    1. Be notified in an emergency
    2. Give consent to medical treatment
    3. Give consent to administer medication
    4. Give consent for an educator to take the student off campus

    Please ensure that the nominated people are aware that you have listed them for this purpose.

  • In the instance where parents/guardians reside a long distance from Melbourne, an Emergency Care Contact is required as a Condition of Enrolment. Please nominate a known relative or family friend who resides within the Melbourne metropolitan region.

    In the unlikely event that an Emergency Contact is activated, there are essential understandings within this process:

    1. The emergency contacts are authorised to:
      - Give consent to medical treatment
      - Give consent to administer medication
    2. I understand that if the student is unwell with a condition that is contagious or not possible to manage onsite, the Emergency Contact may be called and requested to assume Duty of Care.
    3. I understand that if the student has medical appointments that are logistically awkward or a longstanding series of appointments that the Emergency Contact may have to be included in the process.
    4. The MGGS Boarding House preferred GP is located at South Yarra Medical Centre. The Epworth (Private) or Alfred (Public) hospitals are preferred, however, Boarding House staff will be guided by medical professionals as to which hospital is recommended. The Boarding House will utilise these medical providers unless parents/guardians explicitly express otherwise.
    5. The allocated primary Emergency Contact is informed of their responsibilities by you (the parents/guardians) so as to avoid any delay in the care of your child.

  • EMERGENCY CONSENT / DETAILS

  • By enrolling this student at Melbourne Girls Grammar you have authorised the School to:

    • Obtain or provide such emergency or urgent medical treatment for the student where the School considers such action to be necessary; and
    • Obtain and approve such urgent and necessary medical treatment for the student considered appropriate in the circumstances if the student suffers from an injury or illness

    However, to ensure that the student receives appropriate care and consideration at all times and to ensure her records are current we also ask for the following consent.

  • IMMUNISATION STATUS

  • All students entering the School must be fully vaccinated for their age in accordance with the National Immunisation Program or on a recognised catch-up schedule or have a documented medical reason for not being immunised.

    Admission to the School will not be permitted unless written evidence of one of the aforementioned documents is provided to the School.

     

    As per the advice on the Australian Government Department of Human Services website, a detailed Immunisation History Statement (IHS) can be obtained by:

    • Accessing your Medicare online account through myGov - you can also use the Express Plus Medicare mobile app
    • Contacting your vaccination provider to print your Immunisation History Statement for you
    • Contacting the Australian Immunisation Register (AIR) on 1800 653 809
    • Emailing AIR on air@humanservices.gov.au to request a copy
    • Going to a service centre

    Please be aware that it can take up to 10 business days to get your statement in the post.

    For privacy reasons, anyone aged 14 or older must get their own statement. They can do this using the same methods as listed above.

  • PERMISSIONS

  • Short Excursions

    This permission allows the student to leave the School campus grounds for any short School excursions (no further than 2km from the school) during the course of the School day or after hours as appropriate. This permission does not cover overnight, half or all day excursions. Separate permission will be sought for these. Not granting permissions for short excursions means that parents will be required to give permission for each short excursion on an individual basis.

    MGGS (and approved contractors) takes photos and films of students for reasons that include:

    • recording student participation in School events and activities
    • celebrating student achievements
    • for prosperity and to maintain historical records of the School's past and present students and the nature and course of their education.

    Photos and video captured for the reasons listed above are used in support of School learning programs. Photos and videos may be published on the following internal mediums:

    • eVI - our internal Learning Management System
    • Seesaw digital photos (ELC)
    • as a means of documenting programs and student reflections (ELC & JY)
    • Messenger – the fortnightly School newsletter where student achievements are highlighted.
    • The Anderson – our annual School magazine produced as a keepsake for students and as a reflection of the current year
    • Official School photos taken annually including class, year level, or group photos.
  • From time to time, MGGS may seek to publish the photos and videos it records for the following reasons:

    • Marketing and promotional initiatives such as on the MGGS website), MGGS social media platforms, digital marketing, campaigns, promotional print or electronic media, school brochures, flyers, posters, and advertising material.
    • Reporting on School initiatives, news, and information about MGGS and student activities and achievements to the MGGS community

    Photos and videos may be published on the following external mediums:

    • MGGS website 
    • External newsletters and magazines. 
    • The Information Exchange (IE) – the biannual online magazine that showcases MGGS, its programs, community endeavours and activities 
    • Social media

    Where the image or video will be used for an extended period in external publications, the family will be formally notified, and additional permission will be sought. 

  • For social media posts through our community facing channels (eg Instagram), the permission granted for records of School events will be deemed satisfactory. No material that could be deemed sensitive will be published. Where there is any doubt, the image/s will not be used.
